Phimosis -what is it?
Phimosis is the inability to retract the preputium downward over the glans penis. This can be the result of adherences to the glans or a fibrotic preputial ring making it impossible to expose the glans penis. It affects most new-borns, but its incidence diminishes with age. At the age of 3 years the incidence decreases to 10%, and at the age of 14 to 1%. Phimosis can be classified into two groups: • Congenital phimosis -present since birth • Acquired phimosis-happens at later age, for example after diabetes.
Symptoms of phimosis:
Apart from discomfort during sexual intercourse in adult life, if the distal preputial ring is too narrow it can cause ballooning during passing urine, making it difficult to maintain penile hygiene, which can be a cause of balanoposthitis (redness,pain,swelling,pus,itching,bleeding,cracking,foul smell) This chronic condition leads to the development of adhesions between the glans and the inner leaf of the prepuce and also the development of a fibrotic ring, severely narrowing the opening of the prepuce. A condition called paraphimosis can also result when the foreskin is retracted, but can’t be moved back into its normal position. This may require emergency medical attention. The treatment of phimosis can be either medicine or surgery. Medical approach: application of a corticosteroid ointment.
Surgery:
1.Standard Circumcision- in which the foreskin is removed with the help of scissors and self-dissolving stitches.
2.Stapler Circumcision (ZSR circumcision)- the foreskin is cut using a stapling device which also stitches the wound with the help of fine stainless-steel staples
3.Laser Circumcision- LASER is also frequently used to perform circumcision inwhich laser energy is used to cut the foreskin and self-dissolving stitches are placed.
4.Other techniques: Plasti-bell Circumcision, Gomco Clamp Circumcision, Sham ring Circumcision
